Gonioscopy-Assisted Transluminal Trabeculotomy as an Option in a Young Myopic Patient with Progressive Glaucoma

چکیده
A 30-year-old man with a history of juvenile open angle glaucoma (JOAG) presented with a complaint of decreased visual acuity (VA) in his right eye in the setting of elevated intraocular pressure (IOP). An optical coherence tomography (OCT) scan and visual field test were performed, with each test showing extensive retinal fiber layer (RNFL) thinning and visual field deficits respectively. Upon ocular examination, both optic nerve heads were found to have significant cupping with significant peripapillary atrophy and high cup to disc ratios. Due to the man’s young age and profound visual field defects, aggressive treatment was suggested and he underwent gonioscopy-assisted transluminal trabeculotomy (GATT) on his right eye. The procedure successfully lowered his IOP by greater than 30% from baseline, and his visual acuity returned to baseline in the right eye. This is significant considering IOPlowering is the basis for glaucoma treatment and with the advent of this effective new conjunctival-sparing procedure, long-term adverse effects and the need for further glaucoma surgery is reduced.

Minimally invasive glaucoma surgery: current status and future prospects
Minimally invasive glaucoma surgery aims to provide a medication-sparing, conjunctival-sparing, ab interno approach to intraocular pressure reduction for patients with mild-to-moderate glaucoma that is safer than traditional incisional glaucoma surgery. The current approaches include: increasing trabecular outflow (Trabectome, iStent, Hydrus stent, gonioscopy-assisted transluminal trabeculotomy...

متن کاملCiliary Body Tuberculosis Presenting with Attacks of Acute Intraocular Pressure Rise
Purpose: To report a rare presentation of ophthalmic tuberculosis. Case Report: A 19-year-old man was referred to the glaucoma clinic due to multiple attacks of increased intraocular pressure (IOP) in his right eye. IOP of the right eye was 50 mmHg, and glaucomatous damage of the optic nerve was present. In gonioscopy, an oval-black lesion was present in the ciliary body band on the temporal si...
متن کاملProgressive myopia in early onset chronic angle closure glaucoma.
A 19-year-old girl presented with advanced unilateral chronic angle closure glaucoma and myopia with gross cupping and field loss in a previously hyperopic eye with a marked increase in corneal curvature. This emphasises that a marked myopic shift may be an important sign of glaucoma in a young patient.

PURPOSE To report a case of anterior segment ischemia (ASI) with rubeosis iridis after circular buckling surgery in a highly-myopic patient which was successfully treated with a second intravitreal bevacizumab injection. METHODS Case report and review of the literature. DISCUSSION ASI is a rare but potentially serious complication of posterior segment surgery. Finally it leads to neovascula...
